The DA Form 4856 is a form used by the Department of the Army. It is a Developmental Counseling Form. This form is used to document counseling sessions given from counselors to soldiers in the Army.
What are examples of event-oriented counseling?
Examples of event-oriented counseling include, but are not limited to:
- Specific instances of superior or substandard performance.
- Reception and integration counseling.
- Crisis counseling.
- Referral counseling.
- Promotion counseling.
- Transition counseling.

Do NCOs get monthly Counselings?
Soldiers (E-4 and below) should receive counseling monthly. Counseling for Soldiers is normally recorded on DA Form 4856 but other formats are allowed for the initial counseling. NCOs (E-5 and above) should receive counseling quarterly unless their conduct requires it more frequently.
